R5 adds the ArtifactAssessment Resource to attach comments, classifiers, ratings, questions and responses to an existing resource while keeping separate attribution and rights; it exposes 19 StructureDefinition elements and supports referencing parts via path, contained resources, and canonical/URL links. Implementations on the FEvIR platform show it powering formal term voting, a my ballot tool that creates one ArtifactAssessment per vote, compound MeSH qualifiers, and complex risk‑of‑bias judgements, and it is already used successfully for voting, classification, and critical appraisal.
